Trip-A-Day Contest Rules
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
HOW TO ENTER
a. The Trip-A-Day Giveaway Contest (the “Contest) will begin on Monday, October 5, 2009 at 12:00am Pacific Time (“PT”) and will continue to run until announced by KTWV-FM. (the “Contest Period”).
b. To participate in the contest, you may enter via the following method:
Listen to KTWV-FM Mondays through Fridays between February 2, 2009 and TFN. Between the hours of 8:00am-6:00pm PT each day, KTWV-FM will play a designated sound effect of an ocean wave.
When the designated ocean wave sound effect plays, be the 20th caller number to 1-800-520-WAVE (9283). The 20th caller as determined by the KTWV-FM on-air personality will be declared as qualified to be in the running for that day’s Trip-A-Day giveaway, pending confirmation of eligibility. At the time of their call, each 20th caller must provide his or her first and last name, complete address including zip code, telephone number, and date of birth.
A total of eleven (11) Qualifiers will be in the running for the Trip-A-Day Grand Prize giveaway each weekday during the Contest Period. Every weekday between the hours of 7:00pm and 8:00pm PT KTWV-FM will announce the one (1) Grand Prize winner from that day’s Qualifiers only.
c. KTWV-FM is not responsible for telephone service outages, delays, busy signals, equipment malfunctions and any other technological difficulties that may prevent an individual from completing his/her telephone call.
d. For those listening to the Station stream on the Internet: Please be advised that you may be listening to a delayed stream of the radio signal, which may vary depending on your computer’s memory capacity and your Internet connection. For that reason, we strongly recommend that you turn on your radio to the Station to participate in this or any other call in contest to be sure you are participating in “real time.” The Station assumes no responsibility for contestants not making timely phone calls to the Station as a result of the delays in the Internet Stream.
e. Only one qualifying entry permitted per person per weekday.
ELIGIBILITY RESTRICTIONS
a. The contest is open to KTWV-FM listeners who are 18 years of age or older and who reside in the following Southern California Counties: Los Angeles, Orange, Riverside, San Bernardino and Ventura. Employees of sweepstakes sponsor KTWV-FM (the “Station”), its licensee, its parent corporation, subsidiaries and affiliates, their advertising agencies, participating sponsors/promotional partners, other radio stations in the Los Angeles metropolitan area and the members of their immediate families and/or households are ineligible to participate or win. Employees of promotional participant Pueblo Bonito properties and their immediate families are also ineligible to win.
b. Only one winner per household is permitted in any Station contest. Listeners may only win one prize every ninety (90) days or every six (6) months if the prize value is $1,000 or greater.
c. Grand Prize winner must possess valid government-issued photo identification in order to receive prize.
d. Listeners can only qualify once per week during the Contest Period.
e. All qualifying names do not carry over from weekday to weekday. Listeners must try to
re-qualify to be in the running for the Contest for each new weekday.
f. Grand Prize winners can only win one (1) trip during the entire Contest Period.
PRIZES
a. A maximum of 20 Grand Prizes will be awarded. Each Grand Prize winner will be given the choice of receiving one (1) of the following:
i. A prize of either two (2) prize certificates which may be used for roundtrip air transportation for two (2) from Los Angeles International Airport to the destination of the winner's choice anywhere in the world or from the suggested 199 destinations below, subject to availability, blackout dates, and other restrictions. Class of tickets will be based on the discounted contracted class fares through ticket consolidators of All Star Travel Group. Not valid on published fares unless the published fare is lower than the contracted fare.
ii. A check in the amount of $1,000 in the winner's name from KTWV-FM.
iii. One (1) five (5) day/four (4) night stay for two (2) people in a Deluxe Ocean View Room at the Pueblo Bonito Pacifica Holistic Retreat and Spa in Los Cabos, Mexico or in a Junior Suite at Pueblo Bonito Sunset Beach Resort and Spa Cabo San Lucas, B.C.S., Mexico.
b. Winner will have two (2) days within initial contact from KTWV-FM to make their decision as to which trip option they choose.

d. IF WINNER CHOOSES OPTION 3A1 AS STATED ABOVE (The 2 Prize Certificates):
EACH WINNER MUST COME TO THE KTWV STUDIOS WITHIN 2 WEEKS OF WINNING THEIR TRIP TO FILL OUT THEIR RELEASE OF LIABILITY FORM AND W9 TAX FORM, AND TO DESIGNATE THEIR CHOICE OF DESTINATION, WHICH CANNOT BE CHANGED WITHOUT COST TO THE WINNER. FAILURE TO DO SO WITHIN 2 WEEKS OF WINNING WILL RESULT IN FORFEITURE OF TRIP. UPON COMING TO THE KTWV STUDIOS, WINNER WILL RECEIVE A COPY OF THE RULES, A FAQ SHEET, LIST OF DESTINATIONS WITH COST AND CONTACT INFORMATION FOR THE ALL STAR TRAVEL GROUP.
SOME TRAVEL RESTRICTIONS MAY APPLY. TRIP MUST BE USED WITHIN ONE (1) YEAR FROM THE DATE THAT THE PRIZE IS WON AND IS SUBJECT TO TRAVEL RESTRICTIONS. VALUE OF TRIP IS BASED ON DESTINATION CHOSEN BY WINNER. APPROXIMATE RETAIL VALUE IS BETWEEN $500 and $3,000 PER WINNER, AND WILL VARY BASED ON DESTINATION, DATE OF TRAVEL, AND FARES IN EFFECT AT TIME OF TRAVEL.
ALL WINNERS ARE GRANTED 30 CALENDAR DAYS FROM THE DATE THEY RECEIVE TRAVEL AGENCY CONTACT INFORMATION FROM KTWV TO CONTACT ALL STAR TRAVEL GROUP AND BOOK THEIR TRIP. ANY TRIPS NOT BOOKED WITHIN THE 30 CALENDAR DAYS WILL BE FORFEITED. ALL STAR TRAVEL GROUP CONTACT INFORMATION WILL BE RECEIVED BY WINNER UPON SIGNING OF RELEASE & W9 FORMS.
HOTEL ACCOMODATIONS ARE SOLELY THE RESPONSIBILITY OF THE WINNER. IF WINNER IS INTERESTED IN SETTING UP A HOTEL STAY ON THEIR OWN ACCORD, ALL STAR TRAVEL CAN ASSIST WINNER IF NEEDED. ALL TRIPS HAVE A MINIMUM STAY OF SATURDAY NIGHT AND A MAXIMUM STAY OF 30 DAYS. STOPOVERS ARE NOT ALLOWED EXCEPT FOR CONNECTING PURPOSES ONLY. ALL PRIZE CERTIFICATES AND/OR AIRLINE TICKETS ARE NON-TRANFERRABLE AND NON-REPLACEABLE IF LOST OR STOLEN. ONCE REDEEMED, NO CHANGES CAN BE MADE. REQUESTS TO PROCESS PROMOTIONAL VOUCHERS MUST BE DONE AT LEAST 21 DAYS IN ADVANCE PRIOR TO DESIRED SCHEDULED FLIGHT.
Prize Winners and their travel companions are solely responsible for all applicable taxes, including, but not limited to, ground transportation, hotel, hospitality and airport taxes, telephone charges, meals, gratuities, transfers, spending money and any other expenses not specifically offered herein. The guests of the Grand Prize winners must also be at least 18 years of age or older or be accompanied by a parent or legal guardian. The guests of the Grand Prize winners must travel on the same trip itinerary as winners. Winners and guests are responsible for obtaining passports, visas, or any other requisite travel documentation prior to international travel.
e. IF WINNER CHOOSES OPTION 3A2 AS STATED ABOVE (The $1000 cash):
WINNER’S CHOICE TO RECEIVE THE $1000 FROM KTWV-FM IS FINAL UPON DECISION. THE CHECK IN THE AMOUNT OF $1000 WILL BE MADE OUT IN THE WINNER’S NAME AND IS NON-TRANSFERRABLE AND NO SUBSTITUTIONS WILL BE PERMITTED. UPON INITIAL NOTIFICATION FROM KTWV-FM AFTER WINNER, WINNER MUST COME TO THE KTWV-FM STUDIOS WITHIN 14 DAYS TO RECEIVE A COPY OF THE RULES AND WILL BE REQUIRED TO SIGN A W9 FORM. WINNER MUST SUPPLY KTWV-FM WITH THEIR IDENTIFICATION (I.E. DRIVER’S LICENSE) AND THEIR VALID SOCIAL SECUIRITY CARD. WINNER IS SOLELY RESPONSIBLE FOR APPLICABLE TAXES ASSOCIATED WITH THIS PRIZE. ALL CHECKS ARE SENT VIA USPS REGULAR MAIL TO THE ADDRESS PROVIDED BY THE WINNER. CHECKS ARE SENT APPROXIMATELY 8-12 WEEKS FROM CBS RADIO'S CORPORATE OFFICES. f. IF WINNER CHOOSES OPTION 3A3 AS STATED ABOVE (The Hawaii Trip Option):
Winner will receive one (1) five (5) day/four (4) night stay for two (2) people in a deluxe ocean view room at the Pueblo Bonito Pacifica Holistic Retreat and Spa, Pueblo Bonito Pacifica Holistic Retreat and Spa at Cabo Pacifica S/N Cabo San Lucas BCS 23450, Mexico. Or a Junior Suite at Pueblo Bonito Sunset Beach Resort and Spa in Cabo Pacifica S/N Cabo San Lucas BCS 23450, Mexico. Winner will receive a certificate for hotel stay Certificate for hotel is non-transferable and cannot be sold or exchanged to a 3rd-party. Certificate cannot be exchanged for another prize. Accommodations are on a space available basis and availability and are not valid during certain blackout dates. Certificate are valid until 9/30/10. No extensions on certificate expiration date will be offered. If certificate is lost or stolen, no replacement will be given to the winner and prize will be forfeited. If winner wishes to upgrade their room, it will be based on availability and will be responsible to pay the difference in cost between the prize certificate and the upgrade cost. Approximate retail value of prize: $1,400.00. Change/Cash refund will not be given.
Winner is solely responsible for all fees associated with the prize which include but are not limited to all applicable hotel taxes, all meals, all beverages, spending money, hotel incidentals, ground transportation, hotel and airport transfer. WINNER MAY BE REQUIRED TO PRESENT A VALID CREDIT CARD UPON CHECKIN TO THE HOTEL TO COVER ANY INCEIDENTAL EXPENSES INCURRED DURING THEIR STAY. Winner and his or her travel companion must travel on the same itinerary and are responsible for obtaining all required travel documents.
g. All prizes or prize certificates may be picked up at the office of KTWV, 5670 Wilshire Blvd., Suite 200, Los Angeles, California 90036 during normal business hours Monday through Friday between 8:30am-5:30pm only. Any prize or prize certificate not claimed within fourteen (14) days of winning will be forfeited by the winner. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its arrival. KTWV is not responsible for the safe arrival of a prize or prize certificate.
WINNER SELECTION AND NOTIFICATION
a. Decisions of KTWV-FM management with respect to the Contest are final.
b. When listening to play call in contests: Contestants’ chances of winning are increased by listening to KTWV-FM. If listening via online streaming, contestants are hereby notified that internet streaming is not played in real time. There is a delay from what is heard on 94.7 The WAVE.
c. Up to 11 on-air Qualifiers will be selected as per section 1.b. of these Official Rules.
d. A total of eleven (11) Qualifiers will be qualified for the Trip-A-Day giveaway each weekday during the Contest Period. Each weekday during the Contest Period between the hours of 7:00pm and 8:00pm PT KTWV-FM will select one (1) Grand Prize winner in a random drawing of that day’s qualifiers.
e. Winners need not be present but call-in Qualifiers need to listen to qualify.
f. Qualifiers must be available by telephone the day in which they qualify in the Contest between the hours of 6:00pm-7:15pm PT. At that time, a KTWV-FM Programming or Promotions Department staff member will randomly pick one (1) Grand Prize winner from all of that day’s Qualifiers. The name randomly chosen to be that day’s Grand Prize winner will be called by the KTWV-FM Programming or Promotions Department staff member. If Grand Prize winner does not answer between the designated time, they will still be declared the winner. Winner has seven (7) days to contact the station from the day they win. If no contact is made with the winner within seven (7) days, the prize is forfeited and no other winner will be chosen for that day.
g. Odds of being an on-air Qualifier will depend on the number and order of calls received. Odds of an on-air Qualifier winning a Grand Prize are 1 in 11.
h. Winners must execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form within thirteen (13) days of notification attempt or prize will be forfeited and no alternate winner may be chosen by. To complete release forms, winner must come to the KTWV-FM studios.
CONDITIONS
a. Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.
b. By participating in the Contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, the Station in its sole discretion may require Contest winners (and their travel companions, if any) to sign a liability release, agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability
